Name Change Request [FORMAT]

Rules
1- Your score should be more than 100 in order to apply for name change.
2- Your can only change your name once in a month (4 weeks).
3- You do not need to apply for name change if you are using Clan/Gang Tag on your name.
3- While applying you must follow the proper format given below.
NOTE: Rules may updated further. Make sure you check these rules everyday.
